Let me begin with a heart felt thank you, for helping in this matter. 

There are two steps, both are on line. 

Step 1 Contact Names  
that will have access to your building - keys.  Name of the building, Address, Phones, and up to 3 people names and their best phones.   Shelter ID number is your phone number.

once your done click the OK button at the bottom. YOUR done.

 Step 2 Detail Information Form
This is a fill in the block document form, 

Click on the link, it will download the shelterform.doc    Use the SAVE AS button Give it a new name, YOUR PHONE NUMBER   4199462811.doc

Then using your TAB key, tab to the TOWN Box, filling in the information YOU know.

Once completed then click SAVE, then  e mail the file as an attachment to bob@arc-training.org   If you have any questions call me at 740 398 6844
